Controlled respiration in neurosurgery.

چکیده

A GREAT deal has been written about the requirements of neurosurgical anaesthesia, but little about the use of controlled respiration for these cases. It appears, however, that the use of controlled respiration in neurosurgery has spread throughout various centres in Australia. It is intended here to describe a technique for neurosurgical anaesthesia using controlled positive and negative pressure respiration. Prior to April 1956, an open technique was invariably used at St. Vincent's Hospital, Melbourne, which appeared to give better operating conditions than controlled respiration with intermittent positive pressure. However, since obtaining a Harrington-James respirator an increasing number of cases has been done with controlled positive and negative pressure, until the present when the majority of cases are anaesthetized by this method. It is considered that a soft brain and good operating conditions are more consistently achieved.

Comparison of Anesthetic Techniques on Outcomes of Pediatric Rigid Bronchoscopy for Foreign Body Removal

Background Although both methods of spontaneous respiration and controlled ventilation during anesthesia are safe and effective for managing children with foreign body aspiration, there is no consensus from the literature as to which technique is optimal. This study aimed to determine the outcomes of anesthetic techniques in pediatric rigid bronchoscopy for foreign body removal. Materials and M...

Study of the Effect of Increase in The Rate of Respiration on PaCo2 in the Anesthetized Children

SUMMERY A controlled double -blind clinical trial was performed in 40 normal children, with no underlying pulmonary pathology, who underwent surgery. The objective was to study the effect of increase in the rate of respiration (RR) on PaCo2 in the anesthetized child . The RR of the control group (n=20) RR was set at 40/min. In the study patients (n=20) RR was set at 40/min. For the first 30 mi...

Periodic respiration in erect posture in Shy-Drager syndrome.

Tilt-table polygraphic study in four patients with Shy-Drager syndrome demonstrated periodic apnoea in the erect posture. In one patient reduced hypercapneic ventilatory response and necropsy findings of neuronal loss and astrocytosis in the pontine tegmentum suggested dysfunctional respiratory neurones in the brainstem. One patient had Cheyne-Stokes respiration during the late stage of the ill...

The effects of meprobamate and pentobarbitone sodium on sleep and motility during sleep: a controlled trial with psychiatric patients.

Meprobamate (2-methyl-2-n-propyl-1, 3-propanediol dicarbamate) was synthesized by Ludwig and Piech (1951). It is an interneuronal blocking agent capable of producing muscular relaxation without affecting respiration or other vital activities (Berger, 1954). The drug produces restful sleep and generalized muscular relaxation (Borrus, 1955). It was decided to test its effect, compared with that o...

Hyperventilation and the brain – helpful or hazardous ? (

Hyperventilation has been a common practice in neuroanesthesia since the 1960’s. However, a review of the literature suggests that hypocapnia was not initially deliberately introduced into practice to improve operating conditions but was perhaps an inadvertent by-product of muscle relaxation and thereby the need for intermittent positive pressure ventilation (IPPV). The first clinical report of...
